Our Care Assessment and Planning
Creative Caregivers assesses all clients individually and personally. It is important to assess in the home to ensure that we can care for you or your loved one based on our license and their needs. Assessing the client and their history in the home care environment helps us better understand their needs and how we may be able to potentially assist them in the home.
Creative Caregivers believes every client deserves a personalized plan of care. Each plan of care should be developed based on what their needs are, what the family wants to see accomplished, and what the physician has set as goals for their best success and safety, for example change in diet and eating healthy, exercising, and staying compliant with medications. In this plan of care for example, the caregiver would be advised with direction of the family and input of the client if able, what healthy foods to eat so caregiver could grocery shop and prepare, a scheduled walk during the day, and friendly medication reminders. Again, every plan of care is developed based on each client and their needs.
Meeting the potential client helps Creative Caregivers know the person and match a caregiver to that personality. It is critical to have someone that gets along and has similar likes and personalities. That is a key role in the assessment process. Creative Caregivers has been very successful in this function. However, there is always the chance that it won’t be a match, and you can always request someone new. Creative Caregivers believes the best success is with continuity so finding the right match is best for all.
